Standard Operating Procedure for the Management of Samples in the Histopathology Laboratory

1.0 Purpose:

The purpose of this Standard Operating Procedure (SOP) is to establish a systematic procedure for the issuance of blocks, slides, and tissues to customers at ResearchNeeds.

2.0 Scope:

This SOP applies to all sections of the laboratory.

3.0 Reference/Associated Documents:

Please refer to the laboratory's internal reference for additional information.

4.0 Abbreviations:

HPE - Histopathological Examination

IHC - Immunohistochemistry

CCE - Customer Care Executive

SEM - Sample Examination Management





5.0 Instructions:

5.1 Categories of Samples:

Slides (stained/unstained), paraffin blocks, and tissues received at ResearchNeeds are categorized as follows:

For review and HPE

For IHC assays

For Molecular assays

For review by cytopathologist

5.2 Handling of Slides and Blocks:

Slides and blocks from walk-in patients designated for review and HPE are returned upon request (written/verbal). The individual receiving them must provide their signature and name, documented in a register maintained in the Histopathology section/Sample Reception, along with the customer care executive. The storage of retained slides, blocks, and tissues follows SOP no: [Insert SOP Number].

5.3 Return of Samples for IHC Assays and Molecular Assays:

Slides and blocks received for IHC assays, molecular assays, and outstation reviews of HPE are returned along with the corresponding report. A record of this return is made in the section register.

5.4 Retention for Section Reference:

An effort is made to retain a representative slide and/or block in the histopathology section for any future section reference. Blocks/slides are filed according to the department ID assigned to them upon receipt. All returned/issued slides/blocks are recorded in the section block/slide issue register.

5.5 Loaning for Special Programs:

Slides/blocks may be loaned for special programs related to education/proficiency testing. Details of all loaned slides and blocks are recorded in the section block/slide issue register. An attempt is made to retain a representative slide/block for future reference. Upon return, the date is noted in the issue register, and an acknowledgment receipt is obtained from the borrowing institution, following the format specified in [Insert Format].
