STANDARD OPERATING PROCEDURE FOR PREPARATION, REVIEW AND APPROVAL OF ORGANOGRAM

1.0 OBJECTIVE

1.1 To lay down a standard procedure for preparation, review, and approval of Organogram.

2.0 SCOPE

2.1 This SOP shall be applicable for preparation, review, and approval of Organogram of Department of Quality assurance (QAD) and Animal House Facility (AHF).

3.0 RESPONSIBILITY

3.1 Head- QA

3.2 QA Personnel (QAP)

3.3 Head- AHF

4.0 DEFINITIONS

4.1 Organogram: It is a diagrammatic representation of an organization and how the various positions are related to each other. It shows the relationship between all the employees of the organization.

4.2 It is frequently used to show the chain of command and relative ranking of various positions in an organization or department.

5.0 PROCEDURE

5.1 Organogram shows a hierarchical relationship. The Test Facility Management (TFM) is the head of the organization. Below the deputy TFM, there are four separate authorities, each with a separate area of responsibility.

5.2 Organogram of Animal House Facility of -------------is given in annexure: A……………...


5.3 Organogram shall be controlled by the QA department.

5.4 Organogram shall be reviewed yearly.

5.5 If an Organogram is to be changed before review date due to some modification in facility, designations and personnel joining/relieving, etc., then change the Organogram by filling a change control form as per SOP for Change Control Format No.:-F……………………...

5.6 Change control is not applicable in case of schedule revision.

5.7 In case of schedule review, do review within 30 working days of the review date.
