 Checklist of Authorization for Handling of Bio-Medical  Waste

 For the First Application documents

1. Form-II

2. Copy of Valid Membership of CBWTF for treatment of Bio-Medical Waste

3. Copy of Previously issued Bio-Medical Waste Authorization 

4. Bio-Medical waste categorization AS PER SCHEDULE -I of Bio-Medical Waste Management Rules, 2016 along with per day generation as prescribed in Form -II



For Renewal Application documents

1. Application for Authorization (Amendment 2018) Form-2 (Rule -10)

2. Form -1 Consent Seeking Form (Rule-27)

3. Form -1 Consent Seeking Form (Rule-3)

4. CBWTF Certificate

5. CBWTF Agreement

6. CBWTF through issued  barcode in name of the laboratory

7. Following amount DD in favor of Regional Pollution Control Board Office. Renewal Fee for Biomedical Waste Authorization 4000/- per year. It depends on the laboratory if it applying for the upcoming one or for three years. (Green Category Industries)

8. DG Set above 500 KVA charges are 5000/- per year. (Hazardous Waste Authorization), waste oil that remains after half-yearly or yearly servicing need to be sent oil waste management contractual agency and preserve the fee receipt. That company has to share with the pollution control board during inspection or during renewal.

 9. STP Cum ETP (outlet and Inlet) Analysis Report (Latest)

10. Logbook (Copy) for last 3 months of bio-waste generator and disposal provided by the contractual CBTWF
