Sample Examination Management: Issue of Blocks, Slides & Tissues

1.1 Establishing a protocol for the issuance of blocks, slides, and tissues to customers within the research SOP.

5.0 Instructions:
5.1 Categorization of slides (stained/unstained), paraffin blocks, and tissues received in the following categories:
For review and HPE
For IHC assays
For molecular assays
For review by cytopathologist
5.2 Slides and blocks from walk-in patients for review and HPE are returned upon request (written/verbal). The signature and name of the recipient are documented in a register maintained in the Histopathology section/Sample Reception, with the Customer Care Executive. Retained slides, blocks, and tissues are stored following SOP.
5.3 Slides and blocks received for IHC assays, molecular assays, and outstation HPE reviews are returned with the corresponding report. Entries are made in the section register for proper documentation.
5.4 Efforts are made to retain a representative slide/block in the Histopathology section for future reference. Blocks/slides are filed based on the department ID assigned upon receipt. All issued/returned slides/blocks are recorded in the section block/slide issue register.
5.5 Slides/blocks may be loaned for educational/proficiency testing programs. Details of all loaned materials are recorded in the section block/slide issue register, with an attempt made to retain a representative slide/block for future reference. Upon return, the date is noted in the issue register, and an acknowledgment receipt is obtained from the borrowing institution in the specified format.
