What is the treatment procedure for Cochlear Implant in India?

If you’re looking for cochlear implant surgery for your child, you have to go through the following procedures and terms.

A cochlear implant is a small, electronic device that can provide a sense of sound to a person who is profoundly or severely deaf. The implant consists of an external portion, a processor that easily sticks behind the ear, and a second part that is surgically placed under the skin connected to the inner ear.

Cochlear implants bypass the damaged part of the ear and stimulate the hearing nerve directly. Cochlear implants enhance the clarity of sounds and improve your child’s listening ability to understand speech. It may also help you to regain confidence in social situations, make friends and family, and live a fuller life.

In a recent study, people with cochlear implants could understand sentences eight-ten times better than they could previously.

Selection of the right place for surgery and speech therapy:

In India, the172 hospitals are approved by the government for cochlear implant surgeries. Please see the below link. http://adipcochlearimplant.in/ADIP_PDF/list_of_hospitals_17072019.pdf

What are the tests needed to know the exact hearing loss and severity?

The following tests are prescribed by ENT or Pediatric doctors for diagnosis of hearing loss. For these tests, the ENT doctor sends the child to the Audiologist.

BERA (Brainstem Evoked Response Audiometry)

ABR (Audiometry Brainstem Response)

ASSR (Auditory Steady-State Response)

OAE (Oto-Acoustic Emission)

MRI (Magnetic resonance imaging)

Temporal lobe CT (computerized tomography) scan

In case of severe and profound hearing loss, ENT doctors mainly suggest cochlear implants. After the decision to the cochlear implant, ENT generally sends the child for speech therapy, hearing aids trial, and also for psychological evaluation. Vaccination is preferred to do before surgery, to prevent future Meningococcal infections. (Menactra Injection from Sanofi and Prevenar 13 Injection from Pfizer are generally prescribed).

ADIP Scheme: In India, the ADIP Scheme is in operation since 1981, according to this scheme, the government of India provides one ear cochlear implant to economically backward families, the monthly income should not be more than 15000 rupees, and the child should not more than 5 years.

The following documents are mandatory for cochlear Implant surgery under ADIP scheme:

Demographic Information

2 Passport Size Photograph

Birth Certificate (Issued by the City Civic center)

Aadhar Number/ Pan card number (of Mother and Father)

Annual Family Income Proof (Issued by Tehsil Office)

Disability Certificate (From district hospital of the father's residence state, Child Aadhar card is compulsory to register disability certificate)

Audiological Evaluation

BERA (Brainstem Evoked Response Audiometry)

ABR (Audiometry Brainstem Response)

ASSR (Auditory Steady-State Response)

OAE (Oto-Acoustic Emission)

Speech and Language Evaluation

Hearing aids are recommended to use before cochlear implant for 90 days, It is necessary to sound recognition and preserve the residual hearing.

Psychological Evaluation

Radiological Reports

MRI (Magnetic resonance imaging) and Temporal lobe CT (computerized tomography) scan, for MRI and CT Scan, please collect CD or soft copy of radiological images for MRI and CT Scan.

Details of Additional Disability if any

Vaccination Timing

ENT (Otorhinolaryngology expert) role: Recommendation from an ENT expert is mandatory.

Other than the above-mentioned procedure your ENT doctor may ask you to consult a pediatrician, Pediatrician may ask for the following reports before giving the clearance for surgery are as follow: (Not Mandatory)

TORCH Test (Suggest if hearing loss was caused by a virus infection, If a first child is found with hearing loss then there is the possibility of subsequent delivery may have again hearing loss)

ECG

Ultrasound

TFT

Serum Creatinine Level

Clearance from Ophthalmologist.

Additional Information: Functional magnetic resonance imaging (fMRI) is a technique used to obtain functional information by visualizing cortical activity. fMRI detects subtle alterations in blood flow in response to stimuli or actions and also evaluates the plasticity of neurons. This method can predict the success rate of surgery in a child's hearing before cochlear implant surgery. In India, fMRI is performed at the Centre of Biomedical Research (CBMR), Lucknow.
