Quality System Procedure: (PTL/QSP/20) Procedure for Reporting the Results in Testing Laboratories

1. Purpose: To define a system in the organization for reporting the test results.

2. Scope: This procedure is applicable to all tests being conducted in the laboratory.

3. Responsibility: Technical Manager is responsible to implement this procedure. 

4. Procedure:





4.1 The test results are reported at least the following information -

(a) A title “test report”

(b) Name and Address of the laboratory

(c) Unique identification no.

(d) Name and address of the customer

(e) Identification of the method used

(f) The date of receipt of test items

(h) Test results with measurement units

(i) Name(s), function(s) and signature(s) of person(s) authorizing the test report

4.2 The test reports are given to the customer accurately, clearly, unambiguously and objectively.

4.3 Test reports are issued and transmitted to the customer by email, telephone, or other electronic or electromagnetic means.

4.4 All reports are signed by the authorized signatory.

4.5 One copy of the test report is kept in a file for future reference.
